Family Assistance Program

Please complete the attached application and financial snapshot: ensuring you include as much detail as possible. Incomplete applications will not be processed at all due to volume of applications received.

The more detailed your explanation of service history, situation, and scope of needs the faster your case can be reviewed and adjudicated. Please provide a complete list of your duty stations, point of contacts, as well as contact information for anyone who can speak to your needs. Additionally, proof of service (LES/DD214) will be required as well as proper documentation (ex. Current bills) from your scope of needs. ALL INCOME MUST BE SUBMITTED WITH EARNINGS STATEMENTS. Once you have all the requested information please email or fax the appropriate documentation. All information must be filled out on the application. All incomplete applications will not be processed. You must have completed packet, DD214/LES, spouses earnings statement, copy of current bills to be considered complete.

Please be sure to outline your most pressing needs. Our goal is to provide financial assistance to as many military families as possible; as such the Armed Forces Foundation provides a SINGLE ONE TIME grant to qualifying families, again to repeat, that is a ONE TIME ONLY GRANT. This is to ensure that many families in need are able to benefit from this and not just a handful.

If necessary, we believe that clients should consult a financial counselor to create a plan for financial independence.  Please include the contact information of your financial counselor, if applicable.

If you or your spouse is currently unemployed, please submit your resume, so we may help you find employment.


Pre-Qualifications:
Current active duty service member (including Activated Guard/Reserve to active duty) or Departed/released service member within the last year and one half with proof of service. (AIT/Basic training and drill does not quality)

Proof of Extreme Financial Strain Hardship
Copy of VA Disability Letter, if Applicable


Essential Needs***(Items that we can help with):

  • Rent/Mortgage (Civilian housing only)
  • Utility bills (electric, gas, water, sewer)
  • Housing and/or airfare to transport a family member to assist a service member from surgery
  • Childcare during illness or surgery
  • Car payments when it is the family’s only means of transportation (proof required)
  • Car Insurance and registration payments

Items that WILL NOT be considered and not be paid for:

  • Cell phone bills
  • Airfare for vacations
  • Personal loans
  • Personal credit cards
  • Any and all forms of personal debt
  • Legal fees
  • Medical bills (dental included)
  • Airline tickets to visit family
  • Parking tickets
  • Child Support
  • Cable/Satellite Television
  • Storage Buildings
  • Relocation/moving expenses
  • Help with buying a vehicle
  • Security deposits on lease agreements
